I have been having prostate problem for te past 6 years. I first took uroxotrol and it didn't work, then I took Flomax and it didn't work. For the past 3 years I have been taking Flomax at bedtime and Proscar in the morning and I was fine until the end of February when I started having in irritation and a sensation before and after I urinate, In the first week of March, my psa showed 5.6, since then I have been taking Bactrim F 160mg-800mg twice a day and Levofloracine 500mg once every 24 hours. I has a psa test done this week and it read .66, now I still have problems, not as bad, the sansation is gone,but I still have the irritation before and after I urinate. When I wake up and start my day I feel fine and as the day progresses I start getting the irritation. Do I keep taking the medicine or what can I do. Please Help.

Hello. I will try to help. It seems you have a benign prostate hypertrophy. Flomax and Proscar are the best treatment for that (Flomax helps voiding the bladder more easily and with a better flow, and Proscar slows prostate growth). In March, in appears you had a minor and frequent complication of the prostate hypertrophy, a urinary tract infection. The treatment of the infection (with two antibiotics) seems to have worked, since you feel better now. I don't think the slight symptoms you have now are due to persistence of the infection, because you took two good antibiotics and for enough time (at least one month). Maybe the mucosa of the urethra is still a little irritated from the infection. I would perform new cultures (or urine, preferably after prostate massage to try and get prostate secretions with the urine). If the culture is negative I don't think it would be warranted to repeat the antibiotic therapy. I think it would be better to continue with Flomax and Proscar, and I would add an anti-inflammatory (i.e. ibuprofen) for a few days. I would also have an ultrasound and uroflowmetry performed.
